What data is collected?
At the first appointment information such as your name, D.0.B, address and contact telephone numbers will be taken.
Medical history and any medications you are currently taking will also be noted as this will ensure a better understanding of your overall health, how this may be impacting on your foot health and any contraindications treatment may have on medications you're taking.
If consent is not given for this information to be collected KD FOOTCARE will be unable to carry out any foot care assessment or treatment.
Where is my data stored?
Once you have registered as a KD FOOTCARE patient all information that is gathered is stored electronically on cliniko. This is an electronic cloud based database for health records. Access is password protected and cliniko will not share, sell or rent any personal information with anyone unless ordered by a court of law.
Who collects the data and is responsible for its safe storage?
Kate Davies mobile foot health practitioner and owner of KD FOOTCARE is responsible for the safe storage of all data and is the only person who can access this data.
How will it be used?
The information collected by KD FOOTCARE will only be accessed by Kate Davies foot health practitioner to record treatment sessions or to find contact information such as address or telephone number.
No information that is held by KD FOOTCARE is shared with any other organisation apart from referrals to other health care professionals which will only be done on the permission of yourself.
How long are records kept?
Information is legally required to be kept for 8 years after treatment has ended.
After this time period all records held on cliniko will be deleted.
Can I see my records?
Yes, all clients have the right to see their records. Please email Kate Davies kdfootcare@gmail.com
